Here you will find the needed forms that the NCCYC district and the parishes will
use. Most of the forms are either in Microsoft Excel or Microsoft Word format. If
there is a form missing that you need, please contact us and we will try our best
to get you the form you need and add it to the web site.
1) NCCYC Roster - This is the form that every parish team needs to fill out and turn into
the district. This form lists all the players and coaches that make up the team you are
registering to play.

2) NCCYC Supplemental Roster - This form is used to add or subtract players or coaches
to a team after the original roster has been turned in.

3) Team Entry Form - This form is used by the parishes to enter a team into the league.

4) Team Entry Form / Roster Form Instructions - This is the basic instructions and
guidelines on how to fill out the team entry form and the youth roster form.

5) Background Check - This is the form that must be filled out and turned in to each
person's parish. This is one part of the checklist that all coaches, volunteers and people
that are going to work with children.

6) Coach / Roster authorization form - This sheet needs to be turned in when rosters are
turned in as it proves that all coaches have completed the requirement set by the district.

7) Change of Parish - No Move - This form is used by a player that requests the ability to
play for an old parish that they used to belong to.

8) Change of Parish - Move - This form is a request by a player to continue to play for the
parish that they just moved away from.

10) High School Release - This form is used to allow a high school player to be released
from their parish, that can supply a team, and play for another parish.

11) Over-age / Play Down Form - This form is used to request that a player be allowed to
play for the team based on the grade they are in even when their birthday puts them in a
higher division.

12) Spirit Game Player Form - This form is used to turn in the coaches and players that
are being selected to participate in the Spirit games.

13) Roster Review Guidelines - This sheet is the basic guidelines on how to verify that the
roster form has been filled out correctly and accurately.

14) Player release form - This form is used to allow a player to play for a different parish /
school even though they do not attend that school, or are a member of that parish.

15) Score Protest Sheet - This sheet is used to protest the score that has been recorded
on the official score card. This would be done if the official score card is believed to have
an error. (round #, winning team, final score)

9) Parish Activity List - This form is turned in by the parish reps each sport season listing
all parish activity that is to be scheduled around.
